Sign up for a Library Card

If you are a Vanderburgh County Resident and have never had an Evansville Vanderburgh Public Library card, complete this form to register on-line as a new borrower of our library. Required fields are marked with an asterisk (*). Once registered, you will receive a confirmation number which can be used to place up to two holds. To obtain a library card for checking out materials, accessing databases, or making computer reservations you will need to go to one of our library locations within 30 days and show a picture ID and proof of address. A parent or guardian must accompany any child under the age of 18 and the adult's ID may serve as address verification for the child.

• A borrower may check out materials on his/her card only.
• A borrower is responsible for the care and return of all library materials borrowed on his/her card.
• A borrower is responsible for notifying the library if his/her card is lost or stolen. If not reported a borrower is responsible for all materials loaned on his/her card.
• If a borrower does not pay fines or respond to the final overdue notice, his/her account may be turned over to a collection agency with a SERVICE CHARGE.
• Failure to return material and/or pay charges may affect a borrower's credit rating and/or result in litigation.
• If the registrant is under age 18, the signing parent/guardian assumes all responsibilities associated with their child's borrowing privileges.
• If legal guardianship changes, it is the responsibility of the signing parent/guardian to notify the library.
