In 2025, EVPL completed 1,259 passport applications! Our trained staff are experts at guiding people through this process.
Recently, we have had an increase in people coming to the library to complete their passport application who learn that they had paid for an online service that didn’t cover their actual application fees.
There are several for-profit websites, ranking high in popular web searches, that charge individuals to fill out their passport application. The service then directs people to EVPL or other passport agents to complete the process. The applicants then find out that they still need to pay their Passport Fee and Execution Fee to EVPL or other agents.
The applications people are paying for from third-party websites are available to be printed for no charge below; the linked form is the official passport application provided by the U.S. Department of State.

From applying for your first passport to updating your passport photo, EVPL provides you with the opportunity to register for your passport and take your first steps toward worldly exploration.
Appointments
Appointments may be scheduled by calling 812.428.8396 and leaving a detailed voicemail that includes your name, contact information, and availability.
Application Requirements
When applying for your first passport, or when renewing a passport expired for more than five years, you must do so in person. If you wish to renew a passport expired less than five years, you may do so through mail or online.
Required Documents:
- Completed and unsigned application | Download your application
- Two passport photos
- Proof of U.S. Citizenship
- Current proof of identity
Additional Requirements
If you will be providing a photograph for your passport, you will need two identical, whole-face view photographs taken within the last six months. Photographs should be 2″ x 2″ in size and against a light background with no hats or hair obstructing the view of your face.
Fees
Passport and execution fees are due at the time of passport registration.
